Proudly displaying each CCI center’s certificate of accreditation are prestigious and expert partners CCI was fortunate to have help with accreditation. JCAHO will revisit the agency for re-accreditation in three years. With partnerships and teams well established, CCI is ready for its return!


CCI's patient case manager addresses the entire CCI staff gathered for a day-long training prior to the JCAHO survey team official visit. The internal JCAHO leadership team met weekly for a year to plan, implement, analyze and evaluate CCI practices and health care environment to ready the centers and staff for accreditation review. Intensive work was the order of each day in the 11 priority service areas: patient rights and organizational ethics; assessment of patients; care of patients; education of patients and family; continuum of care; improving organization performance; leadership, management of environment of care; management of human resources; management of information; and surveillance, prevention and control of infection.


Staff leaders attend the national Health Disparities Collaborative conference in Nashville. CCI is one of 16 participating agencies in the northeast cluster, and receives cutting edge training and information on diabetes for treatment and disease management education for patients. The affiliation provides a model for clinicians and support staff and an important complement to the JCAHO plan and process.

 

 

 

CCI Receives JCAHO Accreditation

Community Clinic received national accreditation by the Joint Commission for Accreditation of Healthcare Organizations (JCAHO) on August 2006 for three years. This achievement was the culmination of almost three years of preparation and hard work at every level of CCI. Leadership and involvement in the process was provided by the Board of Directors and executive staff, and daily work and change was managed at the health centers.
