Provides primary care services for the low income people in medical need residing in Montgomery and Prince George's Counties
• Four clinics: Gaithersburg, Silver Spring, Takoma Park, Greenbelt (Family Planning only)
• Physicians and mid-level providers will serve 13,500 patients this fiscal year
• Well and sick care for children, adults, seniors, homeless, disabled
• Women's health, family planning, specialty referral, mental health, outreach/care management, Medicaid eligibility screening, specialty referrals
Operates the Women, Infants and Children (WIC) Program for nutrition education and food supplement
• Six clinics: Gaithersburg, Germantown, Aspen Hill, White Flint, Takoma Park, and Greenbelt (WIC and Health Services co-located at Gaithersburg, Takoma Park and Greenbelt)
• Will serve 35,000 participants this year
• Largest Maryland WIC provider since 1996
• Over one-half of pregnant women in Montgomery County receive WIC services
• Screening participants for post-partum depression and Medicaid eligibility
CCI Staff (WIC and Health Services)
• Over 120 staff persons including administration; 80 percent bilingual and
over 20 different languages
• Health services has 13 full and part-time physicians (7 family practitioners,
5 pediatricians, 1 rheumatologist); 4 physician assistants; 1 nurse practitioner;
2 LCSW; and one certified diabetes educator
• WIC has two registered dieticians and 40 certified nutrition counselors
Patients:
• 80 percent Hispanic/Latino; 15 percent African American
• Payment sources: Medicaid (52.5 %); Self-pay (28); Montgomery Cares (11); Medicare (4.7); Care for Kids (3.8)
