Call the clinic of your choice for medical services and ask to make an appointment. The patient representative will ask if you have health insurance. If you don't have insurance, you can still receive medical care at the clinics. The patient representative will give you information about the CCI sliding scale of reduced fees for service and help determine eligibility for you and your family. The patient representative will make the appointment for you, ask you to bring in some proof of income, and ask that you come early to ill out forms. The clinic staff will call you the day before your appointment to conirm you are coming. If for any reason you must cancel, please call the clinic the day before to let us know and to reschedule.
