POSITION SUMMARY
The Family Nurse Practitioner (FNP) provides professional medical services in the community health center setting. Under the direction and supervision of the Medical Director, the FNP will provide comprehensive medical services including family planning to patients of the health center. The FNP will collaborate with other collaborate with the Medical Director in the management of patients and coordinate referral and follow-up care.
SPECIFIC DUTIES AND RESPONSIBILITIES:
• Demonstrates satisfactory knowledge and clinical judgment to provide appropriate care for clinic patients
• Performs general physical examination as indicated and instructs patients in health promotion and disease prevention.
• Examines patients to determine presence of illness, utilizing physical findings, laboratory results, medical history, ascertains nature and extent of illness and prescribes treatments as indicated.
• Documents and maintains patient records of services provided according to agency and JCAHO standards.
• Develops and institutes appropriate treatment plans per protocol with the consultation of Medical Director. Treatment will include the care of acute and chronic medical problems as well as contraceptive services.
• Assists with various counseling and education functions in areas such as birth control, family planning, sexuality, gynecological problems and safer sex.
• Participates in the development of protocols.
• Establishes a collaborative relationship with other medical providers.
• Participates in continuing education courses, in-services and staff meetings. Attends other meetings as assigned.
• Participates in Performance Improvement and Peer Review.
• Productivity satisfactory and consistent with the agencies goals.
• Responsible for other related job duties as assigned by the Medical Director.
EDUCATION AND EXPERIENCE:
• Current MD License.
• Current Family Nurse Practitioner certification.
• Spanish preferred.
• Cultural sensitivity required.
• At least 1-2 years experience working with underserved, low-income persons or homeless persons preferred.
• A strong commitment to serve homeless and medically underserved families.
WORKING CONDITIONS
• Medical/Clinical environment.
• ADA: This position may require long periods of standing. This position also requires the ability to drive between clinical sites in order to provide additional coverage when needed.
