Family Support Coordinator

POSITION SUMMARY

The Family Support Coordinator will be responsible for working with uninsured patients in the community health center assisting staff with case management and administrative support services.

PRINCIPAL DUTIES AND RESPONSIBILITIES

1. Conduct face to face interviews with patients to do intake assessment, which includes: enrolling and/or referring patients into Montgomery Care, Sliding Fee program, Care for Kids, and other programs.
2. Assist patients to complete initial registration forms.
3. Contact uninsured patients for enrollment and conduct a follow-up.
4. Responsible for providing ongoing follow-up with patients and working closely with medical provider to determine whether patients have comprehensive and coordinated care. Follow-up should be continuous from initial identification through case closure.
5. Works with clinics, Montgomery County Department of Health and Human Services, and CCI staff in building multi-cultural competencies with CCI.
6. Make referrals to community agencies as appropriate.
7. Responsible for being an advocate for the patients and maintaining a networking relationship with other social services (i.e. Department Human Services, Heath Care for Homeless, Medicare, hospitals, support groups, etc.).
8. Work with other clinical personnel assigned to the same patient.
9. Responsible for providing consistent communication to the Medical Director, ensuring that information, and reports are provided to clearly describe case management progress.
10. Produce Monthly progress/program reports.
11. Provide staff support and facilitate the coordination of community events, health fairs, and other related health education activities programs, particularly tailored to the needs of the community.
12. Responsible for performing the following types of duties on a daily basis:
13. Exhibits excellent guest relations with patients, visitors and staff, effectively communicating CCI's mission.
14. Mandated to attend regular meeting internal and external meetings.

REQUIRED SKILLS OR ABILITIES

1. Requires experience and demonstrated abilities for working in a multi-cultural setting.
2. Knowledge of some medical terminology.
3. Ability to collaborate and interact with a diverse group of health care professionals.
4. Ability to initiate and maintain working relationship.
5. Have understand and knowledge of population and community of service area
6. Good community skills: able to listen well, use language appropriately, and able to communicate in writing.
7. Excellent customer service skills

REQUIRED KNOWLEDGE, EXPERIENCE OR LICENSURE/REGISTRATION

1. High School Diploma or its equivalent, some college is preferred.
2. Bilingual in Spanish and English a plus

WORKING CONDITIONS

1. Work involves moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.
2. Standing and /or walking for more then eight (8) hours per day.
3. Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
4. The employee will be working within a controlled enclosed environment daily.
