One of the oldest and largest providers of
primary care to medically under-served people in Montgomery and northwestern Prince George's counties, CCI offers two major health services programs. In the primary care program, CCI serves more than 10,000 children and adults who are economically disadvantaged, homeless, disabled, with state medical assistance coverage or uninsured. Women, Infants and Children (WIC) nutrition education and food supplement program enrolls more than 30,000 pregnant and breast-feeding women and children under the age of five. Over half the pregnant women in Montgomery County receive CCI-WIC services. Together, the programs serve more than 45,000 people in need who count on CCI for medical and health-related care. CCI is a patient-centered medical home for a significant number of individuals, families, friends and neighbors in our community.
Primary care services include well, acute and chronic care for individuals, including immunizations and flu shots, women's health, family planning, diabetes education and management, mental health counseling services, discount lab and testing services, discount prescriptions, and outreach/care management. CCI providers can also refer patients for specialty care.
CCI patients and CCI-WIC participants can get staff assistance with state Medicaid eligibility and enrollment at CCI clinics. The Family Support Coordinators are bi-lingual and speak English and Spanish. Other language needs can be met as well.
All CCI services are available equally to all patients regardless of ability to pay. CCI-WIC services are free to participants.
